1. Passport: Original Passport with the validity of minimum six months from the date of return and minimum two blank pages for visa stamping. Note: Attach copy of first & last page of the passport along with all old passports (if any).

2. Visa Application Form duly filled and signed

3. Photo Specification: Two recent passport size photographs with matt or semi matt finish, 80% face coverage, white background and without border (Size: 35mm x 45mm). Note: Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.

4. Covering-Letter : Original Covering-Letter from the applicant stating his name, designation, passport number, expenses clause and purpose & duration of visit in brief. The letter should be duly signed by the applicant in blue ink only and addressed to- Consulate General of Brazil, Mumbai. Note: Covering-Letter should be on Letterhead if applicant is Self Employed or on plain paper if Employed.

9. For Family Visit: Original Notarized Invitation Letter in Portugal and English language from Brazilian host along with the copy of Passport, visa, Residence Permit, Employment proof & bank statements.

10. Refusal letter: If you have been refused a visa by an Embassy or High Commission in the last 2 years: Written explanation about reason for trip and refusal.

11. Hotel Confirmation: Hotel Confirmation including the name of the hotel, address, phone numbers and hotel booking code

12. Travel Insurance: Travel Insurance covering the duration of the stay in Brazil

13. Vaccination certificate copy with Passport number

14. Original YFC (Yellow Fever Certificate) at the time of travel

15. Financials: 

1. Original personal Bank Statement for last 03 months till date with Bank seal & signature.

2. Copy of personal  ITR for last 3 years .

3. Salary Slips for last 03 months with company seal. 

16. Proof Of Occupation:

For Self Employed:

1. Company Registration Certificate / Certificate of Incorporation. 

For Employed:

1. Proof of employment like Appointment Letter and Original leave letter from company.

5. Invitation Letter & Agenda: Original Notarized Invitation Letter from Brazilian host / inviting company in Portugal and English language with their CNPJ number, stating a brief about the company, applicant's name, designation and purpose & duration of stay along with the sponsorship details. The letter should be duly signed by the authorized signatory in blue ink only with his name & designation. 

6. Appointment letter from the employer: Copy of an appointment letter from the employer

9. Medical: Applicant has to carry Original YFC (Yellow Fever Certificate) at the time of travel.

c) Salary Slips for last three months with company seal.
