1. Passport: Original Passport with the validity of minimum six months from the date of return and minimum two blank pages for visa stamping. Attach all old passports (if any).

2. Bolivia Visa Application Form to be duly filled & signed by the applicant.

3. Photo Specification: Three recent passport size colour photographs with matt or semi matt finish, 60%- 80 % face coverage, with clear blue background and without border (4cmx5cm)

4. Covering Letter : Original Covering-letter from the applicant on company’s letter head stating his name, designation, passport number, purpose and duration of visit. The letter should be signed by authorized signatory with company stamp and addressed to – The Visa Officer, Embassy of Pluri national State of Bolivia, New Delhi.

7. Documented Economic Solvency for last 6 months

8. Invitation letter from legal person with address in Bolivia if any

9. Police Clearance certificate is required.

10. Medical: Original YFC (Yellow Fever Certificate) has to be submitted along with the visa application & applicant has to carry the same at the time of travel.

11. Hotel Confirmation: Hotel Confirmation including the name of the hotel, address, phone numbers and hotel booking code

12. Travel Insurance: Travel Insurance covering the duration of the stay in Bolivia

13. Vaccination certificate copy with Passport number

Financials: 

1. Original Personal Bank Statement for last six months with Bank's seal & signature.

2. Personal ITR for last 3 years.
