1. Original passport minimum 03 months validity after the expiry of the applied visa and require 2 full blank pages. Attach all old passports (if any). Copy of first and last page of current passport.

2. Completed and signed application form duly filled and signed on column 37 and main column.

3. Photo Specification: Two recent passport size photographs with matt or semi matt finish, 80% face coverage, white background and without border (Size: 35mm x 40mm). Should not be older than 6 months. 

4. Original Covering letter from the applicant, explaining the purpose, name, passport number, duration of the journey and the guarantee to cover all costs that may arise during the stay in Belgium. it should be address to – To, The Visa Officer, Consulate General of Belgium, Mumbai. In case of self-employed, Covering letter should be on company’s letterhead with company seal.

6. Hotel confirmation: for the entire period of stay in Schengen area. Please Note: Hotel should match the ticket dates and maximum stay should be in Belgium

7. Proof of Professional activity

8. Ticket: Round-trip flight reservations (A detailed itinerary detailing each location to be visited and duration of stay at each location)

9. Travel Insurance: Travel Insurance covering the duration of the stay in Belgium

10. Vaccination certificate copy with Passport number

Financials:

1. Company’s and Personal ITR for last 3 years

2. Three months salary slip with Company seal.

3. Company and Personal Bank Statement for last 03 months mentioning the Bank’s name, Bank’s Telephone Number, bank seal and signature on all the pages.

Note: Company's financial documents are required in case of self employed.

Proof of Occupation:

Self Employed: Company registration copy/ Memorandum /Partnership deed

Employed:  Original leave letter from Employer with leave period, applicant's details and should be signed by authorised signatory with his name, designation and company seal.

Student: Leave Sanction Letter from school. NOC/ ID copy of school/college.

In case of Minor:

If the minor is travelling alone then Original notarized NOC by both the parents along with the copies of passport or ID is required. (NOC should be on Rs.100 stamp paper)

If one of parents is accompanying the minor then an Original notarized NOC by the other parent along with the copies of parents passports or ID is required. (NOC should be on Rs.100 stamp paper)

6. Invitation letter from the Belgium based Company stating the exact purpose and period of the visit and should be duly signed with signatory’s name and designation.

6. Hotel confirmation: for the entire period of stay in Schengen area. (in case of overnight halt)

7. Confirmed ticket reservation with all flights confirmed proving the transit port of entry is Belgium

8. In case of a minor who has not reached the age of 18 years AND is travelling with one of the parents only or alone: a notary affidavit mentioning the consent of the parents must be submitted. This document needs to be apostilled. (The same should be on Rs.100 stamp paper)

6. Copy of proof of payment of the contribution covering administrative costs.

7. Copy + original of your working permit/professional card

8. In case of working as an employee : original of work permit + 1 copy (the work-permit or exception of work-permit has to be applied by the future employer in Belgium with the Regional Ministry of Employment)

9. In case of working on a self-employed basis : original of professional card + 1 copy

10. Medical certificate (+1 copy) attesting that the applicant is not carrying any of the diseases which might endanger public health

11. Certificate attesting to the lack of any convictions for crimes or offences under common law (+1 copy).

12. A Police Clearance Certificate for the past 1 year. This document is valid only 6 months from the date of issue. In case you resided in a foreign country during the past 1 year, you should submit a Police Clearance Certificate issued by the relevant authorities of that foreign country.

Note: You should submit your application minimum 30 days before the foreseen travel date and You can submit your application at the earliest three months before your planned visit.

Invitation letter : A letter of invitation letter from family or friend stating relationship and inviting you to Belgium +proof of relationship
